Learn about the Healthy Indiana Plan Health Insurance

Logansport Memorial Hospital and Area Five are sponsoring information sessions about the Healthy Indiana Plan (HIP), a subsidized health insurance for low-income, uninsured adult Hoosiers. The program offers comprehensive health benefits, including physician services, mental health services and prescription drug coverage. The program is subsidized by the State, and therefore, requires only minimal monthly contributions for the participant.

Five sessions will be held in Conference Room D/E at Memorial Hospital: • Dec. 4 - 1:00 p.m. • Dec. 11 - 8:30 a.m., 11:30 a.m. or 4:00 p.m. • Dec. 13 - 6:00 p.m.

Eligibility requirements include: • Non-disabled adults between the ages of 18 and 64 • Household income between 22% to 200% Federal Poverty Level • Uninsured for at least 6 months • Unable to gain employersponsored health insurance

Each session will include a presentation followed by questions and answers. Help with filling out applications will be provided.
